Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) is a significant provider of high quality mental health services across a core catchment area covering Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. The Trust also provides specialist services for a wider catchment extending throughout the south west. Our aim is to enable and empower people to reach their potential and to live fulfilling lives through providing recovery and reablement focused services that yield positive outcomes for our service users and their carers.

Central & North West London NHS Foundation Trust provide a range of services to treat people with a variety of health needs, including common physical health problems, long-term conditions, mental health, learning disabilities, eating disorders, addictions and sexual health. We are able to provide support for people through every stage of life.
The majority of our services are provided in the community, which means treating people in their homes of from clinics close to home. Where this is not possible, we offer a number of facilities to treat people in hospital or residential environments. We also provide healthcare in prisons in London and the surrounding areas.
We are committed to providing services that meet the needs of the people who use them, and we actively encourage involvement from local people who can help make the difference.

Devon Partnership NHS Trust was established in 2001. We are an NHS organisation that works closely with other health and social care providers across Devon to support the recovery of people with mental health needs.
We are working on making some changes to the way we deliver our mental health services. People who use our services have told us they want them to be clear, simple to access, easy to understand, safe and of high quality. The changes we are making will help us to achieve these aims, and help us to be more efficient.

Dorset HealthCare University NHS Foundation Trust provides integrated community health and mental health, specialist learning disability services, community brain injury, community dental services including community hospitals and prison healthcare.
Most of our services are provided in the local communities, in people’s homes, community hospitals or in local centres through locally based integrated health and social care teams and facilities. We also provide specialist assessment and treatment inpatient centres.

Kent and Medway NHS and Social Care Partnership Trust provides mental health and social care services for Kent in partnership with Kent County Council. In Medway, we work closely with the local unitary authority to provide joined-up health and social care services.

Sussex Partnership NHS Foundation Trust care and treatment for people living in south east England.
Our mental health services provide care and treatment for people with conditions such as psychosis, depression, anxiety, dementia and personality disorder. Our specialist learning disability services provide community and inpatient care for people with complex health needs which can’t be met by other services. We have a number of services that provide very specialist care for people with complex health conditions and social care needs.
We provide care in people’s homes, in specialist clinics, GP surgeries and prisons. Our services are aimed at children, young people and adults of all ages and many are provided in partnership.
