“Datix Huddle” – learning from Incidents

The project is a new way of looking at incidents, giving staff feedback and learning from incidents to improve patient safety. The team meets once a week (huddle) to discuss incidents, break down the incident using CCTV and staff feedback, including how the incident was documented. This weekly huddle also gives staff the chance to give feedback on the incidents reported and they can then explain how an incident was managed. This then creates discussion for the whole team to develop ways of reducing or miniming the risk.

The project has now developed and we invite specialist practitioners to our meeting, when we have an incident that needs more specialist input. For example if we had a patient with a pressure ulcer, we may invite a tissue viability nurse or infection control team to advise us on how to manage the risk.
